No, changing the video settings in the game will not require you to replay the map; they should apply immediately.

It's difficult to describe how to extract the map in a few words. You need to take all the files and folders from the Campaign archive, except for the UserCampaigns folder. From the UserCampaigns folder, take one of the maps, for example, M5, and put it in the Maps - SingleMissions folders. Add all the obtained files and folders to the archive, set the extension to .h5m, and put it in the Maps folder in the game directory.
