Place the downloaded file in the UPDATE folder (which is in the root folder of Heroes). Then, in the root folder of Heroes, run H3wUpd.exe and follow the instructions.

My advice to you: turn off all these extras; they only create imbalance in the game. I play 3.58 because of the updated editor and monsters. P.S. I left the commanders. In my opinion, they don't interfere with the game....
