It also depends on the setting. For example, I haven't played King's Bounty or S.T.A.L.K.E.R., but some kind of jargon is expected there, especially in the latter. In fantasy settings, however, this is not common; "medieval" natives usually don't know about modern expressions.
They speak normally, in the sense that there is almost no difference in pronunciation, but in the subtitles, it personally grates on my eyes.
