If the ones that pop up suddenly while moving are part of the random events system. Ambushes appear at any difficulty level; the higher the level, the more there are. Well, it's balanced that way, and there are some imbalances.

Why can ordinary zombies, skeletons, and ghosts inflict blindness in T-poses?

They don't have the corresponding animation; it's a flaw that you just have to accept.
