It's simpler than you think - you need to use GrannyViewer to see the direction of the axes of the bone to which you're attaching the sword effect, etc. The Z-axis isn't always pointing upwards (the standard orientation for a head or helmet)! Then, in Maya, you need to center the model; for a sword, this is the center of the sword's handle - but the orientation along the axes should be done taking into account the axis orientation you saw in GrannyViewer!
