The survey takes place before the murder, so the Hero would have had time to survey before his death. But I'm more inclined to think that it's Crow :( And the old woman either flew in with a survey into Mental, or Rick blocked her, or she decided to attack with a psycho attack.

Mental isn't exposed. Either he's cunning, or the pepper spray works, or he's just unlucky.
