5. DD/Fly/TP/wings (in my opinion, should be banned)

I agree! On a small map, the chance of finding magic items of level 4-5 decreases, which means they won't break the template in general, and in rare games, "decided to use a scroll of flight," etc., which, in my opinion, is acceptable.
