There's no penalty for not completing it. 3) 100% a bug - I haven't learned how to do that yet. 4) The Witch gives almost all the best artifacts, just in case you want to have some fun. You can create a powerful Dark Mage. Or, for example, get the Sar-Elam set. For an Orc, you can choose the Lion set. (But, to be honest, I've never completed her quest either.) 5) Well, I thought that going to the Ents was something all players would do automatically. And the fact that you need to return to them later was, I think, mentioned... 6) Actually, there are seven of them (underground passages). If you read carefully, the Fifth Crystal is destroyed after completing the quest with the rebels in Grayhill Strike. 7) No, he's just sitting in his city and gathering strength.
