I didn't have any problems with this. I visited all three, and the quest said that none of them were visited, but when I turned it in, the quest was completed successfully. There were also 25 bear riders that needed to be in my party.
Hmm, that's strange, I definitely went to all three. One is at the bottom, in the dungeon, the second is roughly in the middle, and the third is near the water, at the top.
Maybe it's because I had 33 bears instead of 25? I didn't really pay close attention to that line in the quest.
