Петиция к Ubisoft и Limbic

I tried to answer.
and didn't understand.
There are only three pages of questions?
After the third one, I was kicked back to the main menu, without any:
"thank you for participating in the survey, your result has been recorded"
Now I don't know if I did everything correctly, or if it was some kind of glitch and my results weren't saved.
