I can't say for sure who was the first, but the earliest non-biblical mention of Belial that I know of was in 1983 in the Dungeons and Dragons setting. He also appears in Devil May Cry, Painkiller, Final Fantasy, and Warhammer 40K. Sorry for the off-topic.
