The second post in this thread provides detailed instructions on how to write the report header.

Player nicknames must be written exactly as the player is registered on the forum.

There is no player siva in the tournament, and there is no user siva on the HW forum.

The program that calculates the rating does not check user nicknames. Let's not create unnecessary confusion.
