Regarding the mafia's game: they messed up every night, one after another, and the maniac killed everyone.
In the second night, they killed Spira, but he was protected (this was just random, not our mistake), which is why they later suspected him of being the maniac/watcher.
In the third night, it wouldn't have mattered who we chose; the doctor's ultimate ability would have saved them anyway.
