The question is, if I clear the cave completely first, then the orcs and the area around Ludmila, will the undead attack? Or will they quietly wait until I decide to reach the sea?

If you don't touch Ludmila, then maybe no one will go through the orcs to get to you. Try it out.

But in general, leaving enemies in your rear is not the best strategy. I killed the orcs (on my side) in the first week (including the quest one); they only had the fort left. Ludmila became mine and attacked the undead swimming across the sea. Anastasia, meanwhile, killed the demons. Now I'm planning to go and fight the undead.
