All the cities from the list are located in the Dungeon; you can reach them through Inferno portals, and they are all shown in the initial mission.

RANDY_JOHNSON, did you play on Hard difficulty? I wouldn't be able to complete it with such large groups (250 Cave Lords :eek:).
