I didn't quite understand the comment. The second number is the number of villains. 2 - if they didn't injure the surgeon, 3 - if they injured the surgeon or our pharmacist tries.

Do you think they will just kill each other? Is this a potential stroke of luck?

This is one of the possible scenarios. It has the same right to exist as all the others (including an "alliance" with the crazies). An alliance with the crazies is the same as hoping for luck.
