I can't seem to get to the mausoleum (dragons are guarding it, and there are 200k of them; is this intentional, or did it just take me a long time to get there?
And where is the Tome of Chaos? I think it's in the Dungeon?

I've captured all the castles except for Inferno and the Dark Elves.

As stated in the readme, this map is less about fighting (except at the beginning) and more about thinking, analyzing rumors in the tavern, etc. That's why there are a few things (a hero in the Dungeon, 200k hit points, etc.) that you simply can't brute-force. For example, you only encounter the hero in the Dungeon when completing the "Elusive Thief" quest. The Thief isn't a bandit; he shouldn't be fighting. He should be running fast:D
Also, there are many interesting things on the map (superheroes who are bullies in the taverns, for example). Or the way to get the key to the yellow (I think) outpost. One of the surprises awaits you when you find one of the two Tomes of Chaos (not counting the ones you can find in various treasure chests).:p
By the way, most of the castles don't need to be captured; you need to acquire them...
In short, don't treat this map like a typical one – gather an army, kill, loot, collect artifacts, and go kill the next one. Treat it more like an RPG:)
