Yes, to achieve this, you need to complete the game three times on each difficulty level.

There isn't such a requirement for difficulty. It's simply:
1) There's a playthrough counter in the settings (text file).
2) A couple of videos appeared after I edited these settings.

But the thing is, I haven't noticed any further changes, which made me wonder if I had a buggy version (I changed the settings on a freshly installed one), or if this is actually how it is.
