Quite an unusual setting, even considering that it's a post-apocalyptic world (yes, we're used to Fallout).
The images seem a bit psychedelic, some of them are pleasing to the eye, especially the Cylinders, and the roles themselves.

Patch and Five are statuses, like human and non-human, but what does the Five status mean (and is it truly Patch)? Which status does the checker receive, and why is it even needed? Is it to confuse him and the people?). I also understood about the head; it's the second status, like faith.

Mr. Death cannot be killed, but doesn't that apply to executions? I initially thought they were all mana, haha.
