There already is one. It's located in the main section (for some reason). We can move it and edit it.

• The report submission form doesn't work; no matter how many times I submit it, it reports some kind of error.

I am more interested in what error causes this. In my single-player game, something similar pops up every turn. Usually, this happens when trying to save or load the game.
